BSAP Domain Overview
Anatomy & Physiology accounts for approximately 12.0% of the CST exam (18 of 150 scored items), making it the largest Basic Science domain. It covers surgical anatomy by body system, tissue types, physiologic responses to surgery, hemodynamics, wound healing, and anatomical landmarks.
What to Study First
- Wound healing phases - inflammatory, proliferative, and maturation - and the physiology behind each.
- Anatomical landmarks tied to surgical approaches - study landmarks in the context of the procedures that use them, not in isolation.
- Tissue types and their surgical relevance - especially connective tissue, since fascia and suture selection connect directly to closure content.
- Hemodynamics and the coagulation cascade - foundational for understanding hemostasis techniques tested under INTRA.

Question Strategy
- Basic science questions are frequently embedded within clinical scenarios rather than asked as pure textbook recall - study anatomy connected to real procedures.
- Landmark questions often pair a structure with a specific vertebral level or nearby procedure - memorize these level associations directly.
- Do not underestimate this domain because it feels academic - candidates who skip anatomy review because they "already know it from school" often miss application-level questions.
Common Traps to Avoid
- Confusing the order or timing of wound healing phases.
- Mixing up retroperitoneal vs. intraperitoneal organ classification.
- Forgetting that a mature scar only reaches about 70-80% of original tissue tensile strength, even after remodeling is complete.
